Clinical Practice Disposition Assessment (Residency I, Residency II, or Student Teaching)

  • If you are a Residency I applicant, you must enter a total of 2 individual raters as follows:
    1. Junior class instructor
    2. Practicum Public School Teacher
  • If you are a Residency II applicant, you must enter 1 individual rater as follows:
    1. Senior class instructor
  • If you are a Student Teacher applicant, you must enter a total of 3 individual raters as follows:
    1. Graduate level class instructor (Select "Senior Class Instructor" from drop-down list)
    2. Graduate level class instructor (Select "Senior Class Instructor" from drop-down list)
    3. Practicum Public School Teacher
  • Please contact your rater to ask if he/she is willing to complete the disposition assessment prior to submitting your request form. This is an automated system. The rater will receive the request as soon as the form is submitted.
  • DO NOT SUBMIT MULTIPLE REQUESTS FOR THE SAME RATER. If you need a reminder sent or need to make a change to your request, please contact the Office of Teacher Education (teachereducation@tntech.edu)
  • You may submit raters at different times.
  • If you submit your raters at different times, always begin with Rater 1, or else the form will not submit.
  • If you enter raters at different times, please be sure your TNumber is accurate. If it is not, your request will not be updated.
  • You will be notified by email when each disposition assessment has been received.
  • Please BE ACCURATE in entering ALL EMAIL ADDRESSES! You will not receive email confirmations if your email is incorrect and the rater will not receive the request if their address is incorrect.
  • You may view the rating scale.
